About Us

Family Health Center of Marshfield, Inc., Marshfield Clinic and the University of Wisconsin-Eau Claire developed Community Connections Team (CCT) to engage student and community volunteers to be Community Health Workers. The Community Health Workers are housed within the healthcare setting to connect patients with community resources to address underlying social needs that can negatively impact their health.

The process of meeting needs begins during the medical visit. Patients are screened for their social needs and referred to the Community Health Workers for assistance if needed. Community Health Workers discuss the patient-specific needs, then locate and share local, reliable resources with the patient. Patients are able to meet with Community Health Workers before leaving the clinic, or they can discuss their needs via phone calls. Patients receive follow up phone calls from the Community Health Workers until all their needs have been met.

• Utility bills
